Oooops! Yep, my mistake. I think when I first researched this a couple
years ago, those two intersections weren't on the enroute chart (or
maybe I dreamt it ...). I should have checked my facts.

Let me offer another example of this problem - look at Mammoth Lakes
(MMH) GPS 27. The IAF is HUHWY and is not depicted on the enroute
chart. This is a serious mountain area with no clear altitude for the
transistion from the enroute to the approach. In NORDO, you'd have to
wing it using the Grid MORA and the MSA. But that could leave you so
high at the IAF that the approach would be difficult to pull off. A
connection to the enroute chart would have been nice.
