FAA Order 8260.19 paragraph 810a(1) requires, to place the "IAF" designation
behind every initial approach fix entry on 8260 forms in the "From" column.
Further, paragraph 810a(2) requires an "(NoPT)" entry in the "To" column after
that IAF when the PT is not required. Perfect example is found in 8260.19
Appendix 5, page 3.
